First Question: Your file must be an .at3. Don't just rename the file to a .at3, you must actually convert it. Just google it for a conversion tool. ie AAC to AT3.

Second Question: I can't really help you out with this one, though I don't think it is possible to load iRShell after exiting the ISO. I think it may be possible if you have a Custom Firmware which loads iRshell when the PSP boots.
