sounds like there's something wrong with your flash?
not sure bud, but i did it once a long time ago before this option was available in custom firmware. i don't have time to explain right now (storm brewing and have to shut down) but google a program named "psp brew" it's a pbp editor and you can convert 1.5 pbp's into 1.0 or other. With a bit of luck and skill, you can figure out how to merge the two folders ( \prog\eboot.pbp and \prog%\eboot.pbp) into one eboot.pbp file, then you won't need the % folder at all (thus gettin rid of the corrupted extra icon in the menu). unfortunately this way requires editing of every single 1.5 homebrew app so i would suggest trying a reflash since mine works fine O.o
